These forms, in Adobe pdf format*, can be completed on your computer. After completing the forms, be sure to print them out, as they do not automatically get sent to the District. You may also print and complete the forms by hand.

3 forms of Proof of Residency**(click here for the list)

Kindergarten Registration Packet **(fill out all 12 pages)
Health Forms Packet**(Physical is due before the start of school)
Parent Handbook
Student Handbook Signature Form**(fill in one signature page per school building)
